After a short while, patients would fall into a painless, unconscious state, allowing surgeons to perform abdominal and back operations, remove tumors, clean and repair intestinal lesions, suture wounds, and apply medicinal ointments. Most patients could recover fully within a few days.

This medical achievement was far ahead of its time. Effective general anesthesia was not widely applied in Western surgical practice until the 1840s with the popularization of ether and chloroform. Hua Tuo’s systematic anesthetic surgery technology emerged nearly 1,600 years earlier. Due to the loss of his manuscripts, the exact formula of Mafeisan remains unconfirmed. Modern pharmaceutical research speculates that its core ingredients included medicinal plants with anesthetic effects such as datura and monkshood, supplemented with auxiliary herbs and wine to enhance efficacy and absorption. According to authentic records in the Records of the Three Kingdoms and the Book of the Later Han, Cao Cao, the warlord of the late Eastern Han Dynasty, suffered from chronic migraine headaches and employed Hua Tuo as his personal physician for long-term treatment.

Hua Tuo was a free-spirited scholar who prioritized practicing medicine among the people. He was unwilling to be confined to serving a single noble, so he excused himself on the grounds of family illness and returned to his hometown, repeatedly delaying his return to Cao Cao’s mansion. Enraged by Hua Tuo’s repeated evasion, Cao Cao ordered his arrest and imprisonment. The widely circulated plot that Hua Tuo proposed craniotomy to cure Cao Cao’s headache and was accused of treason is a fictional adaptation from the Romance of the Three Kingdoms, with no basis in official history.

During his imprisonment, Hua Tuo attempted to pass on his lifelong medical manuscripts to the jailer to preserve his medical experience, but the jailer dared not accept them for fear of being implicated. In despair, Hua Tuo burned all his manuscripts before his execution. Later, when Cao Cao’s young and talented son Cao Chong died of illness, Cao Cao regretted executing Hua Tuo, realizing that he had lost a rare medical genius who could have saved lives.
